Case Coordination & Support

Case Coordination organizes services for persons 60 and older according to their level of need. It includes an assessment and regular reassessments of persons aged 60 and over for the purpose of developing and monitoring of service plans tailored to the individual’s needs. Case Coordinators arrange for community services while working to enhance informal support systems. Case Coordinators also contact appropriate community agencies to arrange for services as well as evaluate the effectiveness and benefit of services provided.
